If you fill out the contact form on this website, we collect the information you enter: your name, email address, phone number, and the message you write. If you email or call us using the addresses and numbers listed on this site, we receive whatever information you include.
Contact form submissions are stored in this website’s database and are also emailed to our staff. Along with your submission, our forms software records the date and time and the IP address of the device used to submit it.
Like most websites, our web host keeps standard server logs. These typically include IP addresses, browser and device type, pages requested, and the date and time of the request. We use these only to keep the site running and secure.

Appointment requests and the client portal are handled by SimplePractice, our practice management provider, through their own secure systems. When you request an appointment or use the portal, the information you submit is collected and stored by SimplePractice under their privacy and security practices and their agreement with us, not by this website.
If you prefer not to use the online request form, you are always welcome to call or email us instead.
Please do not send detailed health information through the website contact form or by ordinary email. Regular email and website forms are convenient, but they are not secure channels. Tell us only enough to help us route your request — we will follow up through a more appropriate channel.
